FREE 6-week Healthier Living Workshops
For adults with chronic conditions and caregivers:
Take control of your health and feel better!

Healthier Living is an evidence-based Chronic Disease Self-Management Program developed by Stanford University School of Medicine. Workshops are facilitated by trained leaders and focused on participant empowerment, support and skill development to manage chronic health conditions (arthritis, diabetes, high blood pressure, cancer, depression, etc.). Co-sponsored by San Francisco Department of Aging and Adult Services and On Lok, Inc. 30th Street Senior Center.

Refreshments and gifts provided to all participants.
